  1. Future teaching assignment (if known) in 2009-2010 academic year: Please include grades and content area taught. (mathematics, science or any other)
  2. School demographics (If known):  school type (public/private), location (urban/rural/suburban), school; enrollment, and gender (male/female), ethnicity (white/black/Hispanic/others) and economically disadvantaged  student distribution (give %)
  3. Educational background, including degree title(s), name of institution(s) graduated from, and year of graduation for each degree(s)
  4. Future teaching certification obtained and year
  5. Occupational history and a description of special skills
  6. Details of participation in prior professional development programs

1) Considering your professional performance and especially the quality of your instruction, what are your strength and how does this benefit the students?

2) Tell about a situation in which you were very successful. What happened, what did you do, and what was the outcome?

3) What are your most effective techniques for increasing student learning?

4) What are the most important factors in establishing a long-term working relationship with students, colleagues?

5) What is your highest priority goal for post-RET implementing? Illustrate your answer using a specific classroom activity you plan to implement to achieve this goal.


6) If given an opportunity will you like to participate in the RET program for a second year?
